- Complete the Comprehensive Background Form:

Preparing for Your Evaluation Day
- Get a good night’s rest and eat a good breakfast on the day of the evaluation
- Arrive about 10 minutes before the start of your appointment to familiarize yourself or your child with the space.
- For children 9 and younger, it is required for parents to remain in the designated waiting areas for the duration of testing.
- For children, try to avoid using the word “testing,” as this may cause unnecessary nervousness. Explain that children learn in different ways and that the tasks they will be doing will help all of us understand them and their learning better.
- Please put away cell phones during the testing sessions.
- Also, let your child know that they will be meeting with Dr. Edwards alone.

Schedule your final appointment to discuss your evaluation results.
Pay your remaining balance at least 24 hours prior to your feedback session.
You will receive your final evaluation report along with all supplemental materials including support letters, referral list, and resources.

Magnolia Wellness & Psychology does not directly bill commercial insurance, Medicaid, Medicare, or Medicaid Managed Care plans. However, we provide superbills for potential out-of-network reimbursement when applicable.
